2021-2022 Return to School Required Acknowledgments

The information below is for policy and procedure reference only. 
Families are to to complete the live digital Acknowledgement Form version by August 20, 2021.  

The West Allegheny School District is committed to the health and safety of our students and staff.  We have adhered to the health organizations' recommendations for in-person instruction.  Please complete the below acknowledgments.  It is critical that you review the expectations with your child in order to protect them, your family and our West A family at school. 


Mask/Face Covering Compliance Procedures
I understand that my child is required to wear a mask anytime they are on a District the school bus, as per federal CDC order. The West Allegheny School District is recommending, not requiring, masks be worn for all staff and students.  I also acknowledge that, in the event Allegheny County is designated as a substantial or high transmission county based on the CDC, my child will be required to wear a mask/face covering while indoors, regardless of vaccination status.  I also acknowledge that my child will be required to wear a mask if there is any negative impact of COVID in our school(s) and/or if a local, state or federal order is evoked.  I understand that my child will be provided with face covering breaks in the classroom, while they are working independently at their desks and maintaining six feet of social distancing.   

Further, I acknowledge that due to increased risk of viral spread while singing, that my child is required to wear a mask while singing in music and/or chorus class.  

I will provide my child with the appropriate face covering/mask to use daily on the school bus and in school as necessary.  I will provide my child with a face covering that covers his/her nose and mouth and that is secured to the head with ties, straps, or loops over the ears or is wrapped around the lower face.

Mask Exemption
In the event masks are required due to substantial or high community transmission and/or other negative impact and/or local, state or federal order, I am requesting a mask exemption for my child for the reason indicated below.  Please note, a mask exemption does not exempt your child from wearing a mask anytime s/he is on a District school bus.  A mask exemption does not exempt your child from wearing a mask while participating in a class where singing is required.  

These requests will be reviewed, and families will be notified upon approval.

Social Distancing and Hallway Transition Procedures
The West Allegheny School District has provided six feet of social distancing in all classrooms, to the greatest extent possible. I acknowledge that my child will comply with the social distancing measures in place and maintain six feet of distance in his/her assigned seat in all classrooms, in cafeteria spaces, and any other instructional and/or activity space.  In the event six feet of social distancing is not possible (ex. high school science classrooms), I understand that my child must wear a mask while in a classroom that is less than six feet, regardless of status of masking in the District.  There are very few classrooms with less than six feet.  

I further acknowledge that my child will follow all appropriate hallway directional configurations, s/he will not congregate in any hallway or unsupervised location, and will move through the hallway maintaining as much social distancing as feasible.

Home Health Screen
I agree to complete the daily health screen with my child prior to sending him/her to school.  I agree to keep my child home if s/he has one or more symptoms in Group A AND/OR has two or more symptoms in Group B AND/OR are taking fever reduction medication.  

Prior to sending my child back to school after an illness, I agree to call the school nurse to discuss re-admittance to school.

Health Guidelines
I agree to keep my child home if anyone in my home is experiencing COVID-like symptoms and/or is being tested for COVID, unless my child is vaccinated and NOT experiencing any symptoms.

COVID Exposure and/or Return from Illness
I agree to report to a Pandemic Nurse (Barb Lecker [email protected] or Mary Beth Hill [email protected]) any suspected and/or confirmed exposure impacting my child's household.  By acknowledgement, I agree to contact the Pandemic Nurse prior to sending my child to school after any suspected exposure to a positive individual.  Further, prior to returning my child to school after any illness, I agree to call the school nurse to discuss readmittance.

Travel Quarantine Guidelines
If my child is traveling out of the United States, I will notify the school principal and school nurse to discuss whether a 10-day quarantine and/or testing is necessary prior to my son/daughter's return to school.

Use of Disinfectant Spray Guidelines/Procedures  
In an effort to further reduce the spread of COVID-19, the West Allegheny School District will be utilizing a Non-Hazardous disinfectant to clean high-touch surfaces during the school day.  West Allegheny students will be asked to disinfect their desks any time there is a change of class.  Students in grades K-8 will disinfect their desks after being sprayed by the teacher, and 9-12 students will utilize the spray bottles provided and wipe their desks.  

If you prefer that your child/ren not utilize the district-provided disinfectant, you agree to provide your child with household disinfectant wipes to use on a daily basis so they may comply with the disinfectant procedures to reduce germ spread.


I Agree. I will allow my child to utilize a district-provided non-hazardous disinfectant to clean surfaces during the school day. 

I Do Not Agree. I will provide my child with disinfectant wipes for use in disinfecting during the school day. 


Technology Use Agreement for 1-to-1 Device Program
District-issued devices are the property of the West Allegheny School District and should be returned to your child's school in the event your child is no longer enrolled or if the equipment is no longer being used.  If the device is damaged or malfunctions, please contact or bring your child's device to his/her school's main office where you can obtain a loaner device.  The student and parent/guardian will take all reasonable measures to prevent abuse of the equipment and will not lend it to a third party.

I understand that my child will take their device home daily and it is the responsibility of the family to charge the device nightly.
The device is to be used for educational purposes only.  All devices are protected by web-based content filtering that is applied to the device whether it is used at home or on school property.

District-provided Device Insurance Coverage
Insurance coverage is available for your child's device through Worth Ave Group.  The insurance plans include unlimited claims and no deductible: Full Coverage $27 per device for the year; Accidental Coverage $19 per device for the year.  If you elect to NOT purchase insurance coverage, you are responsible for the full cost of replacing or repairing equipment if damaged, stolen, or lost.

Yes, I have purchased the District's provided insurance coverage. I understand that coverage is provided in accordance with the insurance option that I purchased. 

No. I understand that I am fully responsible for the cost of any and all repairs and/or replacements resulting from loss and/or theft. 

Student Code of Conduct
The West Allegheny School District has revised the student code of conduct to include important Pandemic Health and Safety procedures and other required updates.  Please review the revisions with your child.

Transportation Guidelines
I understand that per the CDC order my child must wear a mask/face covering anytime s/he is on a West Allegheny School District bus.  Further, I acknowledge that my child will sit in the seat assigned to him/her.  I have reviewed the Transportation Handbook and associated expectations with my child.

Anti-Bullying/Cybersecurity Policy

I hereby acknowledge that I have reviewed the West Allegheny School District’s Bullying/Cyberbullying Policy and I understand and support the document.  I further acknowledge that any questions I may have had regarding this policy have been directed to and answered by my child's principal.  I understand the importance of a safe and supportive learning environment for all students and will work in partnership with West Allegheny School District administration to ensure that all students are afforded such an environment.

Canine Search Policy
